Knowing how to begin to provide support is important for carers because it helps the carer remain in control and makes the care process more manageable.
Making a plan for care will help both the person with low vision and their support team to clarify the issues that will impact life, identify solutions or service providers and ensure the relevant action is taken. Below is an example of a care plan that you could make.
